Healthcare for Young People Facing Homelessness

Being young should be a time of opportunity, connection, and hope. But for many young people experiencing homelessness, couch surfing, family breakdown, or housing instability, accessing healthcare can feel overwhelming.

At Homeless Healthcare, we provide trauma-informed healthcare and support for young people aged up to 25 who are experiencing or at risk of homelessness. Through our Youth Street Health program and our partnership with Passages Youth Engagement Hub, we deliver healthcare where young people feel safe, welcome, and supported.

Our goal is simple: to remove barriers to healthcare and ensure every young person has access to compassionate, respectful, and ongoing support.

Breaking Barriers to Healthcare

Youth Street Health is a dedicated outreach service focused on improving the health and wellbeing of young people experiencing homelessness across Perth.

With the generous support of the Stan Perron Foundation and JP Stratton Group, Youth Street Health brings together a multidisciplinary team including a Registered Nurse and Caseworker from Homeless Healthcare, working alongside youth outreach workers from Anglicare WA's Street Connect and Vinnies WA's Passages Engagement Hub.

We support young people who are:

  • Sleeping rough

  • Couch surfing

  • Living in unstable or temporary accommodation

  • At risk of homelessness

  • Disconnected from healthcare and support services

Many of the young people we work with are navigating complex challenges, including:

  • Mental health concerns

  • Alcohol and other drug issues

  • Family conflict and relationship breakdown

  • Gender identity and sexuality concerns

  • Neurodiversity-related needs

  • Experiences of trauma, abuse, or neglect

Our team takes the time to build trust, listen without judgement, and provide healthcare that meets young people where they are—both physically and emotionally.

HHC Passages Clinic

Tuesdays 1pm-4pm

Homeless Healthcare operates a regular mobile clinic at Passages Youth Engagement Hub, providing young people with easy access to healthcare in an environment they already know and trust.

Passages is a youth-focused engagement hub that supports young people aged 12–25 who are experiencing homelessness, hardship, or disconnection from their community. The service offers a safe, non-judgemental space where young people can access practical support, referrals, and essential services.

At our mobile clinic, young people can access:

  • General health assessments

  • Treatment for illness and injury

  • Mental health support

  • Sexual and reproductive health care

  • Health education and advice

  • Referrals to specialist health services

  • Support navigating the healthcare system

By bringing healthcare directly to Passages, we reduce barriers such as transport, cost, stigma, and anxiety, making it easier for young people to get the care they need when they need it.

We recognise that every young person's story is different.

Our team works collaboratively with young people to identify their health needs, goals, and priorities. We understand that building trust takes time, and we provide care that is:

  • Inclusive and culturally respectful

We also work closely with housing, mental health, education, employment, alcohol and other drug, and youth support services to help young people achieve broader wellbeing outcomes.
