With the generous philanthropic support of the Stan Perron Foundation and JP Stratton Group, Homeless Healthcare proudly operates a dedicated Youth Street Health team focused on improving the health and wellbeing of young people experiencing homelessness in Perth.

Delivered in close collaboration with Vinnies WA’s Passages Engagement Hub and Anglicare WA’s Street Connect team, the Youth Street Health service provides assertive outreach primary healthcare to young people under the age of 25 who are sleeping rough, couch surfing, or otherwise without stable accommodation.

The multidisciplinary team includes a Registered Nurse and a Caseworker from Homeless Healthcare, working side-by-side with youth outreach and engagement workers from our partner organisations. Together, they deliver trauma-informed, person-centred care that meets young people where they are — both physically and emotionally.

Many of the young people we support face complex challenges, including mental health issues, substance use, gender identity and sexuality concerns, and neurodiversity-related needs. The Youth Street Health team works to build trust, provide compassionate healthcare, and connect young people to the broader support services they need to stabilise their health, wellbeing, and housing.

By working collaboratively with partners across the homelessness and youth sectors, Youth Street Health ensures that vulnerable young people have access to safe, respectful, and continuous care, helping them take the first steps toward a healthier and more stable future.
